This program coordinates the services that care for people with multiple and complex issues.
Who is it for?
This program is for people specially referred by the government's Alcohol and Drug Service (ADS). Referrals can also come from the Burnie Youth Alcohol and Drug Service (BYADS) and Youth and Family Focus (YAFF).
What does it do?
- it works with the person and the ADS to develop a suitable 'care plan'
- it provides a central contact point for all the services involved in the person's care
- it coordinates the care to ensure the most appropriate services are provided
- it supports the person to play an active role in decision-making
