Community Aged Care Packages support older people to remain living in their own homes. This service can provide high-level support including care for the frail aged and people with dementia.
What types of support are available?
We work with you to determine the most appropriate types of support for your circumstances. These may include:
- meal preparation
- personal care
- shopping
- bill paying
- domestic assistance
- transport
- social support
- clinical support from a Registered Nurse
- equipment
- allied health care (such as assistance from an occupational therapist or physiotherapist)
How do I access this support?
To access this support, you must first be assessed by the government Aged Care Assessment team. You can contact us for more information about organising an assessment or ask your doctor, community nurse or staff at a Commonwealth Respite and Carelink Centre on 1800 052 222. Alternatively you can telephone the Aged and Community Care Information Line on 1800 500 853.
Cost?
Up to $30 a week.
