Our clinical support services
In the comfort of your own home we can provide you with services, such as:
- Comprehensive assessments and care planning
- Dementia care
- Palliative care
- Dressings and wound care
- Medication and pain management
- Chronic disease and diabetes management
- Continence support
- Nutritional support and dietary advice, including peg feeding
Your team will work with you to identify what you will need to remain healthy and safe at home.
Anglicare’s clinical team can liaise with your doctor, allied health practitioners, palliative care and other professionals to ensure your support is well coordinated and timely.
If you’ve been admitted to hospital, we will monitor your progress throughout your stay and during your rehabilitation when you return home. We are happy to accompany you to any follow-up medical appointments and hospital tests.
To help reduce your risk of injury, we can suggest special equipment or home modifications.

Anglicare’s Case Managers are committed, clinically trained health professionals who are responsive to your needs.
They partner with you to coordinate your support needs, liaise with medical and allied health professionals and assist you to manage your overall health and wellbeing.
They conduct home safety checks and will discuss with you the full range of supports available to assist you to remain safe at home.
Supporting the role of the primary carer is an important function of our case management services. Your Case Manager will provide on going monitoring, clinical reviews and re-assessment, and adjust your support plan as required.
Your clinical health team ensure that your support is well coordinated and timely in its delivery through liaising with practitioners involved in your care, including:
- Physiotherapist/Occupational Therapist
- Social Worker
If you are admitted to hospital we will monitor your progress throughout your stay, your rehabilitation and when you return home.
To help reduce your risk of injury, we can conduct assessments such as a falls risk assessment. We can suggest special equipment or home modifications such as an electric lift chair or easy access showers.
