Local partnership

A new partnership has been forged to provide local, trusted support to older residents of the Circular Head community.

Older person supported out of a vehicle by a care worker Anglicare is partnering with Smithton-based aged care provider Emmerton Park to deliver Home Care Packages in the area. Home Care Packages support older people with complex needs who need assistance to manage at home. This support is individually tailored to suit each person, and may include domestic assistance, personal care, social support, allied health and clinical services.

“We are both trusted, not for profit organisations with a long-standing commitment to Tasmanians,” said Anglicare’s Connie Bruckard, General Manager of Aged and Home Based Care Services. “By forming this partnership, we are both using our strengths to deliver the best possible services for the community”.

“Anglicare is an experienced, major provider of support for older people throughout the state. Our infrastructure means we bring key management and administration functions to this partnership and we contract Emmerton Park's well trained workforce to deliver the support to clients in their homes”. Emmerton Park also has a residential aged care facility and independent living units in Smithton.

We work with each person receiving a Home Care Package to identify their goals and what services would be most useful to them. “For example, if someone’s goal is focused on maintaining their mobility, their support plan could include an exercise regime with one of Emmerton Park’s exercise physiologists,” said Connie.

“As Tasmanian not-for-profit organisations, both Anglicare and Emmerton Park are committed to delivering an efficient and effective service resulting in lower costs and more direct support for people,” she said.

In addition to community aged care, Anglicare offers a range of other useful services to people living in rural and remote areas of the state. Some public information sessions will be held in the area in early 2018.

Home Care Packages are government funded and determined after assessment by an Aged Care Assessment Team. More information is available from My Aged Care on 1800 200 422. If you would like to speak to Anglicare's Home Care Team please call us on 1800 466 300.
