Supporting safety, connection and care

You are not alone.

The days and weeks after a suicide attempt or crisis can feel overwhelming.  This is a time when extra support can make a real difference.

The Universal Aftercare service offers free, one-to-one support to help you stay safe, feel supported and find your next steps.

How we can support you

Once you are referred, you will be connected with a trained and experienced support practitioner who will:

  • check in with you regularly
  • support you to feel safe
  • help you make a plan for difficult moments
  • listen without judgement; and
  • provide support to family and loved ones if needed.

They can also help connect you with services that meet your needs, such as:

  • housing support
  • relationship counselling
  • financial counselling
  • community groups; and
  • education or training.

This support can be provided in a location suitable to you.

How it works

Referral

Individuals aged 16 and above who have experienced a recent suicide attempt or suicidal crisis are eligible for the service and can be referred by their clinical provider.

Referrals can be completed by:

  • Hospitals and emergency services
  • Mental health professionals.
  • GPs

Operates between 9 am and 4 pm Monday to Friday (excluding public holidays).

A support coordinator will contact you

Our Support Practitioners work during business hours, Monday to Friday between 9 am and 4 pm (excluding public holidays).

They will try to visit the client at the hospital, or ring you the same day.  If the referral is after hours or on the weekend they will call the client on the next business day.

Cost and funding arrangement

The Universal Aftercare service is a free service which is supported by funding from Primary Health Tasmania (Tasmania PHN) through the Australian Government’s Primary Health Networks Program.
