Home and community care program
Our Home and Community Care program provides support to eligible persons in the community who require assistance carrying out activities to manage daily independent living.
Eligibility criteria:
- Under the age of 65
- Living with a functional disability, chronic illness or mental illness
- At risk of needing residential or acute care without HACC program supports
- Requires ongoing assistance to maintain home
Our home and community care services
How we can support
Our staff work within a quality and wellness framework that supports client independence and positive health outcomes.
- Personal care (full assistance or prompting) with personal hygiene
- Assistance with domestic tasks including laundry
- Accompanied/unaccompanied shopping
- Meal preparation
- Social visiting to support mental / emotional wellbeing
- Post-acute care following a hospital admission or period of ill health or treatment
- Work with the client to declutter and clean the home.

Referrals can be received from:
- GPs or other healthcare professionals
- other service providers

The HACC program is funded by the Tasmania Government.
