When you choose Anglicare for your Aged Home Care Package Services, you will be supported by a passionate team of trained professionals who:
You will have your own Care Manager
They will work with you to develop your Care Plan and partner with you to coordinate your support. This includes coordinating medical and allied health professionals. Their job is to assist you in managing your health and wellbeing.
You and your Care Manager will have the support of the Anglicare Scheduling team
They will work to ensure the delivery of your support plan. If a member of your team is on leave or your needs change they will adjust your support for you.
Each Home Care Package provides you with a coordinated set of services to assist you with continuing to live in your own home. The types of services you receive will depend on your individual goals and needs.
There are four levels of Home Care Package funded by the Australian Government, each with a different subsidy level.
Level 1 supports people with basic care needs
Level 2 supports people with low-level care needs
Level 3 supports people with intermediate care needs
Level 4 supports people with high care needs
Your Home Care Package is flexible and allows you to design it to build on your strengths and assist you in achieving your personal goals.
The goal of a Home Care Package is to help you stay independent and continue doing what makes you, you. All from the familiar surrounds of your own home.

Anglicare’s Pastoral Care support service can provide you with spiritual and emotional support during a time of need. They offer a sensitive and caring presence for people of all denominations, including arranging visits to or from your local church or place of worship.
Caring for someone can be physically and emotionally draining. Carers can find it difficult to balance their own needs with those they care for.

If modifications and equipment aren’t funded by your program we can help you to find other funding sources.
During your initial assessment we work with you to select a range of services and products that can be covered within your personal budget.
If you would like to purchase additional services from private or unspent funds you can determine the duration of these services. This means that you only pay for what you need.
You will receive a monthly statement that will outline your income and expenditure from your home-care package. Any unspent funds roll-over to the following month for you to spend later.

You will have the opportunity to apply for some of your home-care package funds to be pre-paid to you on an Anglicare personal debit card. This will enable you to pay for some goods or services when you shop.

Caring for a pet has many physical and emotional benefits, such as decreasing anxiety and depression. A pet also gives structure to the day, with the need for a routine to feed, play, groom and exercise your companion.

Many of Anglicare’s services are funded by government programs, however, we also provide services on a self-funded basis.
These services can also complement any government-funded services that you may already receive.

Contact My Aged Care to be assessed for eligibility for a home care package.
The fastest way is to complete an online application on the My Aged Care Website. This will take approximately 15 minutes.

You can also ring them on 1800 200 422
Once you have put in your application a trained assessor will contact you within 2-6 weeks to arrange an assessment.
They will visit you in your home to discuss your support needs.
Once the assessment is completed and entered in the My Aged Care system, you will receive a letter from My Aged Care with your outcome.
If approved, you will receive one of four levels of funding support. If declined, you can still receive self-funded home support through Anglicare.
You will automatically be placed on the national prioritisation queue (waitlist). You can find out where you are placed within the queue at any time by contacting My Aged Care.
Once a home care package at your approved funding level becomes available, it will be assigned to you. My Aged Care will make contact with you to advise.
You will now be able to choose your preferred service provider and enter into a service agreement.
