Nursing and Allied Health Support Services

Do you need nursing, clinical and care management support at home?

Anglicare’s Aged Care Service has a team of clinically qualified and trained professionals.  They will work in partnership with you and your family to help maintain your health and wellness.

They can offer you a range of services including assessment, care planning and full care management services.

These services can be provided as part of your Home Care Package or as a self-funded home care support.

How can we support you?

Our team will work with you to identify what you need to remain healthy and safe at home.

We can liaise with your doctor, allied health practitioners, palliative care and other professionals to ensure your support is well coordinated and timely.

If you are admitted to hospital, we will monitor your progress during your stay and after you return home. We can come with you to any follow-up medical appointments and hospital tests.

To reduce your risk of injury, we can suggest special equipment or home modifications.

Our aim is to assist you to remain happy, healthy, safe and independent.

In the comfort of your own home we can provide you with services, such as:
  • Comprehensive assessments and care planning
  • Dementia care
  • Palliative care
  • Dressings and wound care
  • Medication and pain management
  • Chronic disease and diabetes management
  • Continence support
  • Nutritional support and dietary advice, including peg feeding.

Care management

Care Manager

Our Care Manager will partner with you to coordinate your support needs.

They will:

  • liaise with medical and allied health professionals and assist you to manage your health and wellbeing.
  • monitor your health, conduct clinical reviews, re-assessment and adjust your support plan if needed.
  • conduct home safety checks and discuss with you the range of supports available to assist you to remain safe at home.
  • support the role of the primary carer.

 

Clinical Health Team

Our clinical health team will work with you and your support staff to ensure that your care is well coordinated and timely.

They will talk directly to your health practitioners such as your:

  • Physiotherapist/Occupational Therapist
  • Chiropractor/Podiatrist
  • Social Worker
  • GP/Specialist

 

Medication Management

Our clinical team can provide you with information about the medication you have been prescribed.  They can ensure that your medication is taken at the right time and record it on your drug chart.

We can support with:

  • Medication reminder products
  • Liaising with your pharmacist and GP
  • Arranging medication reviews
  • Organising medication administration.

 

If you are admitted to hospital our care management team will monitor your progress throughout your stay, your rehabilitation and when you return home.

To help reduce your risk of injury, they can conduct assessments such as a falls risk assessment. They can suggest special equipment or home modifications such as an electric lift chair or easy access showers.
