Choose Anglicare for your Veterans or Commonwealth Home Support Program Services

When you choose Anglicare you will be cared for by a passionate team of trained professionals’ who:

How will we be able to support you?

Once you are deemed eligible your funded support will be outlined and provided to include one or more of the following:

  • Personal Care – Assisting you with hygiene and grooming.
  • Personal Support – Supporting you to undertake certain activities and access your community.
  • Domestic Assistance – Help with domestic cleaning chores to maintain a safe and healthy home environment.
  • Respite Support – Supporting to enable your carer to take a break

 

What if your care needs a change?

If your care needs to be changed we can send a referral to My Aged Care to arrange your re-assessment.

How to choose Anglicare as your support provider?

You must first contact My Aged Care or  the Department of Veteran Affairs to be assessed.  When it is approved and they have outlined your funded supports then you can select Anglicare Tasmania as your support provider.

Be assessed

The first step is to contact My Aged Care (CHSP) or the Department of Veteran Affairs (VHC) to be assessed and approved for home support services.

For CHSP, you can apply for an online assessment or ring My Aged Care on 1800 200 422.

For VHC, you need to ring the VHC Assessment Agency on 1300 550 450

Choose your service provider

Once your funding is approved and your services outlined you can tell the assessment team your preferred provider’s.

However, who you choose will depend on suppliers range of services provided and their capacity to take on more clients at that time. This information can be found on the My Aged Care website for CHSP or when speaking with your VHC assessor.

Once you have decided they will then send your referral to your preferred provider’s, such as Anglicare Tasmania.
