Care Manager

You’ll have Your Own Care Partner

Your Care Partner will:

  • help you create a care plan that suits your goals
  • organise your support, including medical and allied health professionals
  • support you to manage your health and wellbeing

You’ll also be supported by our Scheduling Team:

They will:

  • make sure your care plan is delivered
  • adjust your support if your needs change or if a team member is away

The goal of Support at Home is to help you stay independent and continue doing the things that matter to you – all from the familiar surrounds of your own home.

Anglicare offers a wide range of services. This means we can keep supporting you even if your needs change.

Here’s how we can help

  • Personal Care – support to help you look and feel your best.
  • Social support and community engagement – help you to join in activities and stay connected with people in your community.
  • Domestic assistance – help you with household tasks that may now be hard to do on your own.
  • Other Services – arrange services from other suppliers such as gardening, allied health and meal delivery.
  • Nursing  – checking your health needs, creating a care plan and giving clinical care when needed.
  • At-home respite support – we offer short-term support at home so carers can take a break and rest.
  • End-of-life pathway – we offer extra care and support for people nearing the end of life, so they can stay in their home for as long as possible.

Applying for Support at Home

Contact My Aged Care

Contact My Aged Care to be assessed for eligibility for Support at Home.

The fastest way is to complete an online application on the My Aged Care Website.  This will take approximately 15 minutes.

Visit www.myagedcare.gov.au

You can also ring them on 1800 200 422

In Home Assessment

Once you have put in your application a trained assessor will contact you within 2-6 weeks to arrange an assessment.

They will visit you in your home to discuss your support needs.

Wait for your outcome

Once the assessment is completed and entered in the My Aged Care system, you will receive a letter from My Aged Care with your outcome.

If approved...

You will automatically be placed on the national prioritisation queue (waitlist). You can find out where you are placed within the queue at any time by contacting My Aged Care.

Once Support at Home at your approved funding level becomes available, it will be assigned to you. My Aged Care will make contact with you to advise.

Your Support at Home funding has been assigned

You will now be able to choose your preferred service provider and enter into a service agreement.
